Someone screwed up but that's all the government is saying.
The BC government has completed its investigation in the death of Vernon RCMP auxiliary Glen Evely last November.
Michael O'Brien was being pursued by police in a stolen pick-up truck when he struck Evely's cruiser. Evely's partner, Vernon Constable Francois Grenier, was seriously injured in the crash.
O'Brien is charged with criminal negligence causing death, criminal negligence causing bodily harm, flight from police and theft.
He was mistakenly released from jail in Kamloops and later arrested in Alberta prompting an internal investigation on why he was let go.
BC Solicitor General Rich Coleman says human error is to blame and action has been taken, although he won't say if that includes discipline.
